New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure.
We will no longer accept any new registrations that are outside of our Practice boundary area, and unfortunately if our existing patients move outside of this area they will be removed from our list within 30 days.
You will need to complete a registration form (GMS1) available from Reception and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Also you need to consider whether you would like your computer records to be uploaded on to the summary care record (please see tab 4 for further information)
All new patients are offered a new patient health check with a Practice Nurse to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration. Please contact reception for further information.
You may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
